Telephone Access

If you need to speak with your GP, it is advisable to call and book an appointment. An appointment booking, will allow you the time to discuss any concerns that you may have. Alternatively a message can be passed on to your GP, it is at the GP’s discretion to return your call, or request that you attend the next available appointment.

PATIENT RECALL AND REMINDER SYSTEM

Patients will be contacted regarding test results requiring a follow-up.

Our Practice is committed to preventative care. You may be included on our reminder system. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care. If you do not wish to be part of this system please let your Doctor or reception staff know.

Immunisation information will be passed onto the Australian Childhood Immunisation Register (ACIR) at a National level. 

Important Information

This Practice is a Bulk-Billing Practice. You will be notified prior to your appointment if there will be a cost (eg. commercial drivers licence, private patient). Veterans will be required to sign the service voucher. There is a potential cost when referred for consultation with medical specialists or other health services.

Please ring 02 68375111 for an appointment. Every effort will be made to accommodate your preferred time and GP. Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay or your GP has been called away. Longer consultation times are available, so please ask our receptionists if you require some extra time. 

If you require an Interpreter or have a hearing impairment and need assistance please advise reception while making appointments and arrangements will be made with an interpreter to assist you.

Your medical record is a confidential document, it is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised staff members.

If you have not been into the Practice for a while we may need to update your details on our computer system. The reason for maintaining updated information is very important in order for us to provide you with a high standard of care.

To ensure that we are in a position to provide the best and most appropriate services and referrals for you or your family we require the following information from you at reception-

  • Your full name
  • Date of Birth
  • Current Contact details including address, phone number and emergency contact details
  • If you identify as Aboriginal or Torres Strait Islander origin
  • Your current Medicare card number
  • Your current Health Care card number

When you make your next appointment please take the time to provide reception staff with your updated details so that your medical records remain current.

NOTE – If you identify as Aboriginal or Torres Strait Islander Origin, proof of Aboriginality must be provided.

We take your concerns, suggestions and complaints seriously. However, if you wish to take the matter further and feel that you need to discuss the matter outside of the surgery there are several options available including the Medical Registration Board, Australian Medical Association (AMA) or Health Care Complaints Commission at:

Patients can access up to date information about the practice. At a minimum, this information contains:

  • our practice’s address and telephone numbers
  • our consulting hours and details of arrangements for care outside normal opening hours
  • appointment types
  • our practice’s billing principles
  • a list of our practitioners
  • our practice’s communication policy, including when and how we receive and return telephone calls and electronic communications
  • our practice’s policy for managing patient health information (or its principles and how full details can be obtained from the practice)

New AND Reactivating Patients:

To become a patient of GLAMS, we require the following:

  • A completed New Patient Registration Form
  • A completed Consent to Transfer Medical Records Form
  • Your Confirmation of Aboriginality
  • Your Medicare Card
  • Your Centrelink Healthcare or pension card (if applicable)
  • Your Department of Veterans Affairs card (if applicable)
  • A list of your current medication and allergies

If you have previously visited a different GP clinic you will need to transfer your medical records across to Gilgandra Local Aboriginal Medical Service. This information will assist us in understanding your overall health, your existing conditions, medications, and immunisation history. We will provide you with the documents to complete the transfer of records as part of your registration.

Please be aware that we require your medical record from your previous GP before we are able to offer you an appointment. Some GP clinics do not process these requests promptly, and it may take several weeks for your records to be sent to us.

Once your registration is complete, you will be offered an appointment with an Aboriginal Health Worker or Nurse for an overall check of your health which will then be reviewed by your GP*.

If you have an acute medical condition requiring urgent attention, we kindly ask you to visit your existing provider due to the time required for us to process your New Patient Registration.

How to register:

New (and reactivating) patients will need to register in person at GLAMS Reception

Please arrive with any relevant documents and information to complete your registration forms (please see “what to bring”).

If you or someone you support has difficulties reading and/or writing, staff at AMS will be happy to assist in the registration process.

What to bring:

  • A Medicare Card
  • A Centrelink Healthcare or pension card (if applicable)
  • A Department of Veterans Affairs card (if applicable)
  • A list of your current medication and allergies
  • New patients will be required to bring a copy of their Confirmation of Aboriginality

Parking:
We do not have dedicated patient parking on site however, street parking is available in the surrounding streets.
Gilgandra Local Aboriginal Medical Service is a bulk billing medical service, Our service is dedicated to providing free health care for the community. If you require a referral to an external service provider a fee may apply however, staff will discuss this with you if it is applicable.
